Home care nursing combines administrative experience with direct patient care.

Qualifications:

• Valid RN or LPN/LVN license
• CPR certification
• Minimum of 1-2 years clinical experience
• Valid driver's license and reliable transportation
• Excellent communication and interpersonal skills
• Ability to work independently
• Physical ability to assist patients with mobility
• Proficiency in electronic health records systems (will also be trained on the job)
• Continuing education credits as required by state regulations
• Specialized certifications (e.g., wound care, IV therapy) as needed

Job Duties:

1. Assess patients' health conditions and develop care plans
2. Educate patients and families on health management and disease prevention
3. Coordinate care with other healthcare professionals and services
4. Maintain accurate and detailed patient records
5. Assist with activities of daily living (ADLs) as needed
6. Train and supervise home health aides
7. Participate in case conferences and care plan reviews
8. Adhere to agency policies and regulatory requirements
